We understand that managing your chronic conditions is critical, so our primary care providers offer priceless resources such as diabetes education, group classes, and seamless referrals to endocrinologists when necessary—which all work together to help you keep those numbers right where they ought to be.

The North Memorial Health Advantage

  • Our Diabetes Education program has served the community for more than 30 years by offering high-quality diabetes education services in accordance with the National Standards for Diabetes Self-Management Education Programs.
  • Certified Diabetes Care and Education Specialists can help with a wide variety of things, including: insulin pump management, nutrition counseling, glucose monitoring and interpretation, and direct you to pre-diabetes group classes and an adult diabetes support group, and more.
  • Most insurance companies will pay for all, or a portion of, your customer’s diabetes education for individual appointments. Please check with your insurance carrier for coverage information.
  • For customers whose diabetes requires specialized care, North Memorial Health Endocrinology will collaborate with you and your care team, and assist you with expert, one-on-one guidance and treatment for your specific case.
  • North Memorial Health Endocrinology also assists customers with other diseases affecting the endocrine system, including pituitary and adrenal gland disorders, thyroid disorders, and polycystic ovarian syndrome, to name a few.
